Common Co-Pays Going the Way of the DoDo

One of the basic facts about the cost of health care in this country is that preventive services, such as screenings, have a seriously significant pay-off in the long term. Yet, up until this point, many medical insurance providers have allowed barriers to be placed in the way of these kinds of preventive services. Typically, a co-pay of around $20 or more is necessary for screening for things like high cholesterol levels, diabetes or high blood pressure.

Thanks to the new medical insurance reform law, however, those copays are about to disappear. Beginning on January 1st, most workers will see the co-pay for preventive services go away.

Specifically, the new health care law is aimed at helping employees have access to regular checkups and routine screenings that will, over the long run, reduce the overall costs of health care. It’s much less expensive, of course, to treat a patient before an ailment sets in, or if it’s discovered early.

A hefty cost

The problem is that the law isn’t so much requiring health insurance companies to make changes to their plans as it is requiring employers to make sure those things are covered. What’s happening, then, is that some companies are looking at the prospect of footing the bill for employees as well as their families for these kinds of services.

There are other new costs that employers will be facing, as well. Children up until the age of 26 will have to be covered by group plans that their parents are enrolled in, which will cover children with pre-existing conditions. In addition, the law gets rid of lifetime limits for various types of care.

But will it work?

How much of a different that makes remains to be seen. Workers may find that these important services no longer have a co-pay, but getting them to actually go in and have the necessary tests and such can be difficult, at best. Still, the potential for costs savings not just in the immediate future but over the long haul in terms of the need for advanced medical care suggest that the tactic is valuable, regardless of how many people take advantage of it.

Understanding the Biology of a Hangover

If it was up to us, everyone would drink responsibly and there would be no hangovers to be endured. Unfortunately it isn’t up to us and people tend to get a little out of control. To help you after you’ve made poor decisions, here’s a look at the biology of a hangover, and how to stop the room from spinning.


Competition in the Medical Insurance Marketplace

The conventional wisdom says that, when you have a marketplace for any given product, competition is a good thing. Competition drives down prices, and it also spurs companies to make better products that compete better in the marketplace. One of the big questions facing our nation right now, especially with the sweeping changes to health care regulations that have taken place this year, is whether or not competition is good in the medical insurance marketplace.

One area where the recent legislation might tamper with the competition in the marketplace is in the area of the benefit ratio. Under the law, insurance companies have to pay out at least 80 percent of what they collect in premiums as medical benefits. This has to do with small-group and individual insurance policies. The number for larger groups is even higher, with a requirement that 85 percent of premiums be paid out.

Shrinking the competition

Unfortunately, one of the things that has happened with this change is that some insurers are now getting out of the medical insurance business. They can’t afford to operate at the numbers demanded by the government, so they get out altogether. This reduces the competition in the marketplace, something that is usually bad for the consumer.

Not especially necessary

The new legislation creates medical insurance exchanges in each state, in which medical insurance companies can compete with one another. In that setting, consumers can pick and choose which insurance company they want to do business with. If a company offers the benefits they need, most consumers aren’t worried exactly how their premium payments are allocated. As long as the insurer delivers on their promises, it’s fine for most consumers.

Fast changes reduce competition

This illustrates a bigger problem with the changes we’re facing right now. It isn’t that the changes being made to the medical insurance industry are good or bad (we’ll leave that for you to decide) it’s that the changes are a shock to the system. When changes are put in place rapidly, it creates side effects that can’t often be predicted. A few companies will go under, while others will change the way they operate completely. Ultimately, these fast changes cause there to be fewer and fewer choices in the marketplace.

Flexible Spending Account Rules Changing

These accounts, which are tremendously popular and used literally by millions of Americans each and every year, are under fire. These accounts let you set aside some of your income in a tax free basis. You can then draw money from those accounts to pay for certain types of medical products and services. This is a convenient and useful tool for many folks.

Today, people who have flexible spending accounts can spend that money in a number of different ways. One of the avenues for their medical spending can be on medications that are available without a prescription. These over-the-counter drugs include things like medications for pain relief, medications to help address cold or flu symptoms, medications that help with allergies, or even medications that help with things like indigestion or hemorrhoids.

Beginning in January, however, these medications will not qualify under your flexible spending account. Unless you receive a prescription from a health care provider, you’ll have to pay for those products out of your pocket on an after-tax basis.

There are approximately 15,000 medications and products that this will apply to. Going forward, you’ll need to have a prescription for those products if you want to buy them with money from your flexible spending account.

The stated purpose, according to the medical insurance legislation, is to help raise revenues to cover the costs of the legislation. It isn’t known how much this move will actually further complicate things, as many people will now visit their health care provider for something as simple as a headache so that they can the purchase their aspirin with money from their flexible spending account.

There is a double-whammy in the legislation for flexible spending accounts, as well. Today, there’s no limit as to how much you can put in the account, although if you don’t use it by the end of the year you lose it. Beginning in 2013, there will be a limit to the amount you can contribute to the account of $2,500 per year.

Medical Insurance Theft in Arizona

There’s big money in medical insurance these days. With the rate at which technological advances have hit the medical field, combined with changing regulations as to how medical insurance can work, it’s no wonder that folks find themselves paying higher premiums than they did just a few years ago. Along with those higher premiums comes a significant risk of fraud. From Medicare to private insurance plans, there are scam artists out there who are trying to take advantage of unsuspecting victims as well as the insurance companies.

Most recently, the issue has come to the forefront in a case in Phoenix. There, the Arizona Department of Insurance Fraud identified and has charged a group of individuals for identity theft charges, in which they falsified medical insurance records.

Two people have had charges brought up based on their alleged role in this scam. Prosecutors say that the pair, identified as Gevorg Melkonyan and Elina Arutyunova, stole the identities of as many as 128 patients in the state of Arizona. They then took those identities and billed their insurance companies for a total of over $600,000 in medical services, none of which were provided.

The pair also used the identities of 12 physicians, also who were unaware of what was going on. The insurance fraud unit in Arizona was originally notified by Dr. Scott Bernstein, one of those 12 physicians, who had been contacted by patients. The patients contacted this doctor when they received statements from their insurance companies stating that he’d billed for treatment that the patients never received.

The pair have been indicted on several charges, including conspiracy, illegally conducting an enterprise, assisting a criminal syndicate, fraudulent schemes and artifices, money laundering, three counts of theft, and aggravated taking the identity of another person or entity.

The investigation involved a number of different agencies, including police in both Phoenix and Chandler, Arizona, as well as the Arizona Attorney General’s office, the U.S. Department of HHS, and even the post office.

This situation illustrates the growing concern many folks should have over medical insurance fraud, as well as how identity theft is beginning to infiltrate many different aspects of our lives.

Principal Financial Drops Medical Insurance

One of the natural consequences of the recent medical insurance regulations that have been passed has been the fact that some insurance companies are getting out of the business. One of the biggest companies to do so has been the Principal Financial Group. The group has left the medical insurance industry altogether, and while they cite reasons other than the health care legislation there is plenty of speculation that the decision comes, at least in part, as a response to coming changes.

The issue has become something of a political football. One race for U.S. Congress in Iowa, for example, is becoming a microcosm of what’s happening all around the nation on a larger scale.

In the third congressional district in Iowa, the Republican candidate Brad Zaun is running ads suggesting that Principal Financial leaving the medical insurance field was because of changes in the healthcare bill that was passed this year. That bill was passed in part with the support of the current incumbent, Democrat congressman Leonard Boswell.

Zaun points out that Principal Financial, along with six other insurance companies in that state, have all removed themselves from the medical insurance industry.

A spokesperson from Principal Financial, however, claims that this isn’t the case. Susan Hauser, the spokesperson, claims that Zaun is politicizing the issue.

The issue isn’t completely cut and dried, however. Zaun claims that he’s spoken with other employees at Principal Financial, some of whom were actually involved in the decision to cut medical insurance from their portfolio of offerings, that claim the decision had much more to do with the legislation.

Further complicating the matter is the issue of political contributions. Principal Financial is currently one of the five biggest contributors to the re-election campaign of Congressman Boswell. Whether or not the company is possibly trying to put a positive spin on the situation for that reason isn’t clear.

Ultimately, this is just the first of what is likely to be a number of industry impacts that the new healthcare legislation has on the marketplace. Whether those changes turn out to be more positive or negative remains, of course, to be seen.
